¿como crear un checklist con múltiples opciones?

hola tengo un problema y la verdad estoy muy perdido no se ni por donde empezar

1-no se si se necesiten mas de una tabla y relacionarlas o solo una 2-no se como mandar múltiples opciones de checkbox a la base de datos 3-no se si se una buena idea usar checkbox o hay otras maneras de crear checklist

por ejemplo: en un apartado de mi check list tengo 4 diferentes opciones y de esas cuatro opciones se despliegan otras tres de esta manera:

emblemas bueno/malo/na defensa delantera bueno/malo/na unidades bueno/malo/na direccionales bueno/malo/na

tengo que seleccionar todos e insertar el nombre y si esta en buen estado o no por ejemplo :

  • emblemas-bueno defensa delantera-malo unidades-na direccionales-bueno

y eso insertarlo en la base de datos no se si me entiendan este es mi código: solo e construido los checkbox no se por donde empezar
